Les objectifs de ce travail étaient d’identifier à partir de la TEP pré thérapeutique des paramètres métaboliques quantitatifs pronostics de la survie sans progression et dans un second temps d’établir un score pronostic. Matériel et méthode : Soixante-dix-huit patients, issus de 3 centres différents (Rennes, Lorient et Lausanne), traités par radio chimiothérapie concomitante ou radiothérapie et Cetuximab pour un cancer non oropharyngé localement avancé ont été inclus dans cette étude rétrospective. L’ensemble des paramètres cliniques et TEP a été analysé et une corrélation avec la survie sans récidive a été recherchée. Un modèle pronostic de survie sans progression basé sur les paramètres de la TEP a été défini. Une validation interne a été réalisée par une méthode de ré-échantillonnage (bootstrap). Résultats : En analyse multivariée, le Metabolic Tumor Volume (MTV) est un facteur pronostic indépendant de la survie sans progression alors que les facteurs classiques cliniques (stade T, GTV, AJCC) sont peu ou pas significatifs. Après ré-échantillonnage, le c-index ajusté était de 0.69. La calibration interne a retrouvé un bon ajustement entre la survie sans progression prédite et la survie sans progression observée à 24 mois. Un score prédictif a été calculé, permettant d’identifier 2 groupes à risque. La médiane de la survie sans progression était de 37 mois [IC95% : 36-55 mois] pour le groupe à bas risque et de 8 mois [IC95% : 5-15 mois] pour le groupe à haut risque (p<0.01). Conclusion : Pour les tumeurs ORL de stade III ou IV, la classification AJCC semble insuffisante pour correctement identifier les patients avec un mauvais pronostic. Le MTV est un puissant facteur pronostic de la survie sans progression chez les patients traités par radio chimiothérapie concomitante ou radiothérapie et Cetuximab. L’utilisation du MTV pourrait permettre une identification précoce des patients de mauvais pronostic, candidat potentiel à une intensification thérapeutique. Cependant une validation externe parait nécessaire pour valider ces résultats. Purpose: In the context of non-oropharyngeal locally advanced head and neck cancer (LAHNC) treated with definitive radiotherapy (RT) (combined with chemotherapy or cetuximab), the aims of this study were: (1) to identify PET-FDG parameters correlated with disease free survival from an international multicentric cohort of patients; and then (2) to compute a prognostic score. Materials and Methods: Seventy-eight consecutive patients treated with chemoradiotherapy or RT with cetuximab for LAHNC were analysed to build a predictive model of disease free survival (DFS) based on PET-FDG parameters. After internal calibration and validation by a bootstrap method, a nomogram and a scoring system were developed. Results: In multivariate analysis, the metabolic tumor volume (MTV) of the primary tumor and the lymph nodes were independent predictive factors for DFS, while classical clinical parameters (GTV, stage T, AJCC staging) were less or not significant. Internal calibration showed a very good adjustment between the predicted DFS and the observed DFS at 24 months. After validation by bootstrap, the Harrel’s c-index was 0.69. Using the predictive score, two risk groups were identified. The median DFS was 37 months (95% CI: 36-55) for the low-risk group vs 8 months (95% CI: 5-15) for the high-risk group (p<0.01) Conclusions: For patients with stage III or IV, AJCC staging appears limited to identify patients with a worse prognostic. MTV as a continuous variable was a strong prognostic factor for DFS in non-oropharyngeal LAHNC patients treated with CRT or RT + cetuximab. However, an external validation appears necessary to define its potential interest for tailoring the therapeutic approach.
