<dcterms:abstract xml:lang="en">Background: The health determinants of sex workers are poorer than those of the general population, notably due to difficulties in accessing mainstream healthcare services. Institutional as well as societal stigma marginalizes sex workers. This stigma is also reinforced by healthcare professionals, contributing to mistrust toward the healthcare system. It was therefore necessary to examine the barriers to healthcare access faced by this population in order to improve their overall health status. Local data were collected to ensure adaptation to existing local services. Objectives: To determine how sex workers perceive their access to the healthcare system in the Rennes metropolitan area, in order to identify the obstacles encountered and the impact of discrimination. To propose avenues for improving inclusion and reducing inequalities in access to mainstream healthcare. Methods: A qualitative study based on semi-structured individual interviews with sex workers, conducted between December 2024 and May 2025. Data were analyzed using inductive thematic analysis inspired by the interpretative phenomenological approach. Results: The study highlights multiple barriers limiting sex workers’ access to healthcare in the Rennes metropolitan area. These include internal factors (internalization of stigma, shame, dissociation, comorbidities), societal and relational factors (the burden of stigma and minority stress, relationships with healthcare professionals, mistrust), as well as structural and institutional factors (fragmentation of care pathways, lack of recognition of experiential knowledge, precarious social situations), all of which complicate engagement with and adherence to care pathways. Sex workers have developed protective, avoidance, and adaptive mechanisms in their healthcare journeys. This ongoing adaptation and caution toward the healthcare system justify the need to act on healthcare practices, environments, and professional attitudes. The importance for healthcare professionals of adopting a non-judgmental stance while valuing the autonomy and experiential knowledge of sex workers was also emphasized. The facilitating role of community-based organizations was highlighted. Finally, the study proposes rethinking the traditional medical model in favor of a more transversal approach co-constructed with those concerned, in order to create more inclusive conditions of care. Conclusion: This work underscores the significant impact of stigma on sex workers’ access to healthcare. More broadly, it demonstrates the need to consider all forms of discrimination as factors likely to influence access to care, in order to reduce associated health inequalities.</dcterms:abstract>
