<dcterms:abstract xml:lang="en">Context: Chronic wounds are a public health issue. There is room for improvement, particularly in terms of nutrition, which plays a central role in the entire healing process. What about the continuation of the nutritional strategy in the community for elderly patients with chronic wounds by the attending physician following hospitalization? Materials and Methods: A quantitative descriptive study was conducted using a telephone questionnaire among primary care physicians of patients aged 70 and over hospitalized at the CHBA with a chronic wound diagnosed or monitored during hospitalization and who had received nutritional advice from a dietitian that resulted in supplementation by oral nutritional supplement (ONS). Results: A total of 69 patients were included in the study. Arterial wounds were the most common type. Among all patients, 53.6% were malnourished, 44.9% of whom were severely malnourished. Sarcopenia was not investigated in 63.8% of patients. Weighing on admission and discharge was not systematic, and nearly 32% of hospital records did not include weight data and 26% did not include nutritional status data. In total, 53.6% of general practitioners had prescribed ONSs in the community and 23.2% had recommended dietary supplementation. One-third of patients had no therapeutic strategy in the community. ONSs were significantly more commonly prescribed in outpatient settings for patients with malnourished wounds than for patients with non-malnourished wounds. Dietary supplementation and monitoring of weight and food intake were not more common based on patients' nutritional status or the quality of the hospitalization report. Conclusion: Nutritional monitoring by general practitioners of patients with chronic wounds is not optimal. The prescription of ONS in private practice seems to be more common in cases of malnutrition. It would be appropriate to create a care pathway with systematic nutritional monitoring by a dietitian. The transmission of information upon discharge from hospital should be optimized by improving discharge letters and identifying any obstacles encountered by treating physicians.</dcterms:abstract>
