<dcterms:abstract xml:lang="en">Solid organ transplantation improves survival and quality of life, but long-term success depends on strict adherence to immunosuppressive therapy. Health literacy, the ability to access, understand, and apply health information, may strongly influence adherence. A systematic review was conducted according to PRISMA guidelines. PubMed, Web of Science, and Google Scholar were searched up to March 31, 2025. Eligible studies included adult solid organ transplant recipients with health literacy assessed through validated tools and adherence measured by questionnaires or tacrolimus variability. Eleven studies (n = 1,744) were included, mainly kidney transplant recipients. Limited health literacy was reported in 2.4–24.7% of patients. Seven studies demonstrated a positive association between higher health literacy and better adherence, particularly in kidney and liver transplantation. No negative associations were identified. Health literacy is a modifiable determinant of adherence in solid organ transplantation. Routine assessment may guide tailored educational interventions to improve outcomes.</dcterms:abstract>
