     <dcterms:abstract xml:lang="fr">Cette thèse retrace l’implantation du lambeau de SCIP (Superficial Circumflex Iliac Perforator flap) au CHU de Rennes avec une courbe d’apprentissage initiale (plus de complications) puis une nette amélioration après standardisation du repérage, de la dissection fine/superfine et de la technique générale. Il s’est imposé comme option de premier choix au membre (couverture fine, morphologie adaptée, faible morbidité donneuse) et, en version chimérique (dont ostéocutanée), pour les pertes composites avec d’excellents résultats après échec de greffe osseuse. En reconstruction faciale/intra- orale, nous privilégions des lambeaux fins afin de limiter l’épidermolyse liée à l’œdème, avec renfort cartilagineux si besoin. A travers cette série de 35 lambeaux, nous cherchons à démocratiser le développement des reconstructions par lambeaux de SCIP dans les autres centres. La courte longueur pédiculaire, surtout sur la branche superficielle, impose d’anticiper la stratégie mais ne représente finalement pas une difficulté majeure. En somme, bien planifié et maîtrisé, le SCIP s’affirme comme un véritable « couteau suisse » de la reconstruction.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">This thesis traces the introduction of the SCIP (Superficial Circumflex Iliac Perforator) flap at Rennes University Hospital center, with an initial learning curve (more complications) followed by clear improvement after standardizing vessel mapping, thin/super-thin dissection, and overall technique. It emerged as a first-line option for limb reconstruction (thin coverage, excellent contour match, low donor-site morbidity) and, in chimeric forms (including osteocutaneous), for composite defects— with excellent results after failed bone grafting. In facial/intra-oral reconstruction, we favor thin flaps to limit edema-related epidermolysis, adding cartilage support when needed. With this series of 35 flaps, we aim to broaden adoption of SCIP-based reconstruction across centers. Although the pedicle is relatively short, especially on the superficial branch, anticipating the vascular strategy generally prevents major issues. In sum, when well planned and executed, the SCIP flap proves to be a true “Swiss Army knife” of reconstruction.</dcterms:abstract>
