Résumé : Contexte : La fibrose pulmonaire idiopathique (FPI) est une pneumopathie interstitielle grave, de très mauvais pronostic, caractérisée par un aspect histologique et radiologique de pneumopathie interstitielle commune. De nombreux patients atteints de FPI vont présenter au cours de la maladie, une nécessité d'oxygénothérapie de déambulation initialement puis de longue durée. Objectifs : Identifier les facteurs prédictifs de mise sous oxygénothérapie selon les caractéristiques recueillies au diagnostic chez les patients atteints de FPI et suivre l’évolution et le pronostic des patients atteints de FPI et sous oxygénothérapie de déambulation. Méthodes : Nous avons réalisé une analyse rétrospective de patients atteints de FPI suivis au CHU de Rennes. L'incidence et les caractéristiques de tous les patients atteints de FPI ont été étudiées. Des analyses univariées puis multivariées ont été utilisées pour identifier les variables indépendamment associées à la survenue d'une nécessité de mise sous oxygénothérapie de déambulation. Nous nous sommes par la suite intéressés au pronostic des patients sous oxygénothérapie de déambulation. Résultats : Sur les 364 patients inclus, une oxygénothérapie a été initiée chez 163 patients (44.8%). Les facteurs de risque indépendamment associés à la survenue d'une oxygénothérapie ont été une DLCO réduite (HR 0,98 [0,97 ; 0,99] p<0,0001), une CVF diminuée (HR 0,98 [0,97 ; 0,99] p=0,0007), une albuminémie abaissée (HR 0,95 [0,91 ; 1,00] p= 0.0373) ainsi qu’une CRP élevée (HR 1.01 [1,00 ;1,02] p= 0,0121). La médiane de survie dans le groupe oxygénorequérance était de 27,8 mois. L’oxygénorequerance était associée à une médiane de survie plus courte (p< 0,0001) et à un surrisque d’EA-FPI et d’hospitalisations toutes causes (p<0,0001) Conclusion : Les facteurs de risque de mise sous oxygénothérapie identifiés dans cette cohorte étaient une CVF, une DLCO, une albuminémie diminuée ainsi qu’une CRP élevée. Une fois sous oxygénothérapie de déambulation, les patients présentent un surrisque d’hospitalisations, d’EA-FPI et de décès

Résumé (anglais) : Background: Idiopathic pulmonary fibrosis (IPF) is a severe interstitial lung disease with poor prognosis, characterized by a histological and radiological pattern of usual interstitial pneumonia (UIP). During the course of the disease, many patients develop oxygen dependency, initially on exertion and subsequently at rest. Objectives: To identify baseline predictive factors for the initiation of oxygen therapy in patients with IPF, and to evaluate the clinical course and prognosis of those requiring ambulatory oxygen therapy. Methods: We conducted a retrospective analysis of patients with IPF followed at Rennes University Hospital. Baseline clinical, functional, and biological characteristics were collected. Univariate and multivariate Cox proportional hazards models were used to identify factors independently associated with the initiation of ambulatory oxygen therapy. Survival, acute exacerbations of IPF (AE-IPF), and hospitalizations were subsequently analyzed in the oxygen-dependent group. Results: Among 364 included patients, 163 (44.8%) initiated ambulatory oxygen therapy. Independent risk factors associated with oxygen initiation were lower DLCO (HR 0.98 [0.97–0.99], p < 0.0001), lower FVC (HR 0.98 [0.97–0.99], p = 0.0007), lower serum albumin (HR 0.95 [0.91–1.00], p = 0.0373), and higher CRP levels (HR 1.01 [1.00–1.02], p = 0.0121). Median survival in the oxygen-requiring group was 27.8 months, significantly shorter than in patients not requiring oxygen therapy (p < 0.0001). Oxygen dependence was also associated with a higher risk of AE-IPF and all-cause hospitalizations (p < 0.0001). Conclusion: In this cohort, lower FVC, lower DLCO, decreased serum albumin, and elevated CRP were independently associated with the need for oxygen therapy. Once ambulatory oxygen therapy was initiated, patients exhibited a poorer prognosis, with increased risks of hospitalization, acute exacerbations, and death.
