     <dcterms:abstract xml:lang="fr">Contexte et objectifs: Le score TAM (Transplantation for ACLF-3 Model) et le score SALT-M (Sundaram ACLF-LT-Mortality) ont été proposés pour aider à la sélection des candidats à la transplantation hépatique (TH) atteints d’acute-on-chronic liver failure (ACLF), en vue d’optimiser les résultats post-transplantation. Nous avons évalué la performance et l’utilité clinique de ces scores ainsi que de leurs composantes à l’aide de l’approche du net benefit. Méthodes et résultats: Tous les patients transplantés dans un contexte d'ACLF grade 3 (n = 291), issus de neuf cohortes internationales (2008-2023), ont été inclus dans l'étude. Le critère principal était la survie à 1 an après TH. L’approche du net benefit, qui optimise la valeur prédictive positive des systèmes de score pour une application clinique, a été utilisée pour évaluer les performances des modèles.
Le taux de survie à 1 an après TH était de 77,3 %. Le score TAM (OR 1,88, p = 0,0006) et le score SALT-M (OR 1,10, p &lt; 0,0001) étaient associés au risque de mortalité à 1 an. Un TAM ≥3 (39,1 % vs 79,9 %, p &lt; 0,0001) et un SALT-M ≥25 (46,2 % vs 79,8 %, p &lt; 0,0001) représentaient des seuils à haut risque définis par l’approche du net benefit, associés à une faible survie. Les analyses multivariées ont identifié l’insuffisance respiratoire (PaO₂/FiO₂ ≤200) comme le facteur le plus fortement associé à un mauvais pronostic. Une dose de noradrénaline ≥0,7 γ/kg/min a également été identifiée comme un prédicteur indépendant de la mortalité à 1 an. L’intégration de ces deux facteurs aux seuils du TAM et du SALT-M, basés sur le net benefit, pourrait améliorer l'identification des patients à risque de mortalité à un an après TH. Conclusion: Cette large cohorte multicentrique internationale confirme le potentiel limité des scores TAM et SALT-M pour la stratification du risque chez les patients ACLF-3 candidats à la transplantation hépatique. Les résultats mettent en évidence l’impact sous-estimé de l’insuffisance respiratoire et de l’utilisation de fortes doses de noradrénaline sur le pronostic, des éléments qui pourraient contribuer à améliorer et personnaliser le processus décisionnel multidisciplinaire pour la prise en charge de ces patients.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Background &amp; Aims: The Transplantation for ACLF-3 Model (TAM) and Sundaram ACLF-LT-Mortality score (SALT-M) have been proposed to help select liver transplantation (LT) candidates with Acute-on-Chronic Liver Failure (ACLF) to optimize post-LT outcomes. We assessed the performance and clinical utility of these scores and their components based on the net benefit approach. Approach &amp; Results: All patients transplanted in the context of ACLF grade 3 (n=291) from nine international cohorts (2008-2023) were included in the study. The primary endpoint was 1-year post-LT survival. The net benefit approach, which optimizes the positive predictive value of the scoring systems for clinical applicability, was used to assess model performance. The 1-year post-LT survival rate was 77.3%. TAM (OR 1.88, p=0.0006) and SALT-M (OR 1.10, p&lt;0.0001) were associated with 1-year mortality risk. TAM ≥3 (39.1% vs. 79.9%, p&lt;0.0001) and SALT-M ≥25 (46.2% vs. 79.8%, p&lt;0.0001) were net benefit-based high-risk thresholds associated with poor survival. Multivariate analyses identified respiratory failure (PaO₂/FiO₂ ≤200) as the strongest factor associated with poor outcomes. A norepinephrine dose ≥0.7 γ/kg/min was also identified as an independent predictor of 1-year mortality. Incorporating these two factors alongside the TAM and SALT-M net benefit thresholds may enhance the identification of patients at risk for poor outcomes. Conclusions: This large international multicenter cohort confirms the limited potential of TAM and SALT-M for risk stratification in ACLF-3 patients undergoing LT. Its findings reveal the underestimated impact of respiratory failure and high-dose norepinephrine on outcomes, insights that could refine the multidisciplinary decision-making process for these patients.</dcterms:abstract>
