Environ 15 à 20 % de ces tumeurs surexpriment le récepteur HER2, cible thérapeutique validée dans les formes métastatiques. L’introduction du trastuzumab a permis d’améliorer la prise en charge de ces patients, bien que la réponse thérapeutique reste variable et limitée par des mécanismes de résistance. De nouvelles approches sont en développement, notamment les anticorps conjugués, les inhibiteurs bispécifiques et les combinaisons avec l’immunothérapie. Cette thèse propose une synthèse des connaissances actuelles sur la prise en charge des cancers gastriques HER2-positifs, et discute les perspectives thérapeutiques émergentes dans une logique de médecine personnalisée. Gastric adenocarcinomas remain a major public health concern due to their high incidence and poor prognosis. Approximately 15–20% of these tumors overexpress the HER2 receptor, which has become a validated therapeutic target in advanced and metastatic disease. The introduction of trastuzumab has improved treatment outcomes in HER2-positive patients; however, therapeutic responses are heterogeneous and often limited by resistance mechanisms. Emerging strategies—including antibody-drug conjugates, bispecific antibodies, and immunotherapy combinations—are currently under investigation. This thesis provides an updated overview of current management approaches for HER2-positive gastric cancer and explores promising therapeutic perspectives in the context of personalized medicine.
