Méthode : Etude qualitative par entretiens individuels semi-dirigés effectués entre mai 2023 et janvier 2024, auprès d’IDEL, infirmiers en pratique avancée et infirmiers ASALEE exerçant sur le secteur de Dinan agglomération Résultats : 13 entretiens ont été menés. La relation avec le centre hospitalier de Dinan est globalement considérée comme bonne, mais plusieurs problématiques ont été soulevées : un sentiment de méconnaissance et de manque de considération de leur travail, le manque d’informations concernant le patient ainsi que la difficulté à trouver un interlocuteur pour le lien ville-hôpital. Plusieurs pistes d’améliorations ont pu être avancées, comme anticiper la sortie du patient, améliorer la communication via un interlocuteur unique, recevoir le compte rendu d’hospitalisation, bien qu’il ait été difficile de trouver une méthode ou un outil convenant à tous les professionnels. Conclusion : Les conclusions de cette étude se rapprochent d’étude ayant été réalisées auprès des médecins généralistes. Le rôle de l’infirmier étant amené à se développer, il est important de trouver des solutions pour faciliter leur travail, et permettre une meilleure continuité des soins chez le patient. Objective : To understand the perception of the city-hospital link with the hospital center of Dinan through a new perspective, that of nurses working in peri-hospital settings, and the highlight the levers for optimizing this care coordination. Method : Qualitative study through semi-structured individual interviews conducted between May 2023 and January 2024, with liberal nurses, Advanced Practice Registered Nurses and ASALEE nurses working in the Dinan agglomeration area. Results : 13 interviews were conducted. The relationship with the Dinan hospital center is generalliy considered goof, but several issues were raised : a feeling of lack of knowledge and consideration for their work, lack of information about the patient, as well as difficulty in finding a contact person for the city-hospital link. Several improvement paths have been suggested, such as anticipating the patient’s discharge, improving communication through a single point of contact, receiving the hospitalization report, although it has been difficult to find a method or tool suitable for all professionnals. Conclusion : The conclusions of this study are similar to studies conducted with general practitioners. As the role of the nurse is expected to evolve, it is important to find solutions to facilitate their work and ensure better continuity of care for the patient.
