Or, il a été démontré que le risque de décès augmente avec le temps d’attente aux urgences. Compte tenu de la pénurie de lits en hospitalisation conventionnelle, de nombreux patients attendent aux urgences ou sont hospitalisés vers l'UHCD. Cependant l’UHCD a des critères d’admission propre à la prise en charge des patients des urgences pour des durées d’hospitalisation courte, et non destinée aux hospitalisations conventionnelles. Nous avons donc voulu savoir si l'admission de ces patients dans un service inadapté n'était pas préjudiciable à leur prise en charge. Objectif: Déterminer l'effet sur la mortalité et la durée de séjour de l’admission à l'UHCD de patients en raison d'un manque de lits en hospitalisation, par rapport aux patients hospitalisés directement à partir des urgences. Méthode: Etude observationnelle rétrospective monocentrique menée aux urgences du CHU de Rennes entre le 15 septembre et le 15 novembre 2023. Tous les patients majeurs nécessitant une hospitalisation ou période d’observation ont été inclus. Le critère de jugement principal était un critère composite hiérarchique comprenant le décès à 30 jours et la durée d'hospitalisation. Le résultat secondaire comprenait le taux de reconsultation aux urgences dans les 30 jours. Résultats: Le taux de mortalité à 30 jours était de 10,5 % dans le groupe NUAC contre 5,1 % dans le groupe témoin. Une perte de chance significative a été mise en évidence pour le critère d'évaluation principal composite hiérarchique (mortalité et durée de séjour) pour le groupe NUAC, avec un ratio de victoire de 0,64 (IC 95 %, 0,54 à 0,76) p<0,001. Conclusion: L'admission des patients en attente d’un lit d’hospitalisation à l'UHCD est associée à une augmentation de la mortalité toutes causes confondues à 30 jours, ainsi qu'à une augmentation de la durée du séjour. Background: Overcrowding in hospitals and emergency departments means that some patients wait more than 24 hours in the emergency department (ED) on a stretcher. However, it has been shown that the risk of death increases with waiting time in the ED. Given the shortage of inpatient beds, many patients wait in the ED or are admitted to the UHCD. This service has admission criteria specific to the care of emergency patients for short hospital stays, and not intended for conventional hospitalization. We therefore wanted to know whether admitting these patients to an unsuitable department was not detrimental to their care. Objective: To determine the effect on mortality and length of stay of patients admitted to the UHCD due to lack of inpatient beds, compared with patients hospitalized directly from the emergency department. Methods: A monocentric, retrospective, observational study was conducted in the ED of the University Hospital of Rennes between September 15 and November 15, 2023. All adults requiring hospitalization or observation period were included in our study. The primary outcome was a hierarchical composite endpoint of 30-day death and length of hospital stay. The secondary outcome was the rate of reconsultation in the ED within 30 days. Results: The 30-day mortality rate was 10.5% in the NUAC group and 5.1% in the control group. A significant loss of chance was observed for the hierarchical composite primary endpoint (mortality and length of stay) for the NUAC group, with a win ratio of 0.64 (95% IC, 0.54 to 0.76) p<0.001. In the NUAC group, the proportion of people presenting an adverse event was higher. Conclusion: Admitting patients to the UHCD, while waiting for an inpatient bed, is associated with an increase in 30-day all-cause mortality, as well as an increase in length of stay.
