La fréquence de ce syndrome est d’au moins 1 fille sur 4 500 à la naissance. Afin d’améliorer la prise en charge globale de ces patientes en France, le Centre de Référence des Pathologies Gynécologiques Rares a créé en 2021 un Protocole National de Diagnostic et de Soins (PNDS) pour les aplasies utéro-vaginales. Au CHU de Rennes, un programme de Transplantation Utérine (TU) est actuellement en cours d’organisation. Dans ce contexte, un nombre croissant de patientes atteintes du syndrome MRKH consultent au CHU de Rennes dans le but de participer au programme de transplantation utérine. Afin d’optimiser la prise en charge globale des patientes atteintes de ce syndrome, nous avons souhaité évaluer l’exhaustivité du bilan diagnostique effectué chez ces patientes. L’objectif de cette étude est d’effectuer un état des lieux de la prise en charge globale et notamment du bilan diagnostique des patientes atteintes de ce syndrome consultant au CHU de Rennes. Matériel et méthode : Il s’agit d’une étude observationnelle descriptive, rétrospective, monocentrique portant sur une cohorte de patientes atteintes du syndrome MRKH ayant consulté dans le service de gynécologie du CHU de Rennes du 1er mai 2020 au 31 mai 2023. Les données suivantes ont été recueillies (selon le PNDS pour les aplasies vaginales) : imagerie diagnostique, bilan biologique hormonal, recherche des malformations associées (échographie rénale, radiographie du squelette, audiogramme et/ou consultation ORL, échographie cardiaque), nécessité ou non d’une reconstruction vaginale, préservation de la fertilité et réalisation d’une consultation génétique. Pour chaque item du PNDS, les pourcentages des données qualitatives ont été calculés, pour les données quantitatives la moyenne et les indices de dispersion maximum et minimum ont été calculés. Résultats : 46 patientes ont été incluses. Concernant la recherche des malformations associées, 12,1% des patientes avaient un bilan complet avant d’être adressées au CHU. Le bilan biologique était complet avant la première consultation pour 14,6% des patientes. Les patientes ont eu recours à la reconstruction vaginale dans 68,3% des cas, dont 39,3% ont bénéficié d’une reconstruction vaginale chirurgicale sans information sur un essai de la technique de Franck préalable ou non. La préservation de la fertilité n’avait été proposée à aucune patiente avant leur première venue au CHU. Une patiente avait bénéficié d’une consultation génétique et 2 patientes d’un caryotype lors de leur venue initiale hors CHU. Conclusion : Devant la fréquence de cette pathologie, non rarissime et pouvant impacter la qualité de vie des femmes atteintes, il semble important d’informer les professionnels de santé, particulièrement les gynécologues et médecins généralistes sur le bilan diagnostique à effectuer ceci afin d’optimiser la prise en charge des patientes. Au CHU de Rennes, une consultation ORL avec réalisation d’un audiogramme devrait être proposée aux patientes de manière systématique. Introduction: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is characterized by congenital aplasia of the uterus and the upper two-thirds of the vagina in women with a normal karyotype (46XX). The frequency of this syndrome is at least 1 in 4,500 girls at birth. In order to improve the overall care of these patients in France, the Reference Center for Rare Gynecological Pathologies created in 2021 a National Diagnostic and Care Protocol (PNDS) for uterovaginal aplasia. At Rennes University Hospital, a Uterine Transplantation (TU) program is currently being organized. In this context, an increasing number of patients suffering from MRKH syndrome consult the Rennes University Hospital with the aim of participating in the uterine transplantation program. In order to optimize the overall care of patients suffering from this syndrome, we wanted to evaluate the completeness of the diagnostic assessment carried out in these patients. The objective of this study is to carry out an inventory of the overall care and in particular the diagnostic assessment of patients suffering from this syndrome consulting at the Rennes University Hospital. Material and method: This is a descriptive, retrospective, single-center observational study on a cohort of patients suffering from MRKH syndrome who consulted in the gynecology department of Rennes University Hospital from May 1, 2020 to May 31, 2023. The following data were collected (according to the PNDS for vaginal aplasia): diagnostic imaging, biological hormonal assessment, search for associated malformations (kidney ultrasound, skeletal x-ray, audiogram and/or ENT consultation, cardiac ultrasound), necessity or not of vaginal reconstruction, fertility preservation and genetic consultation. For each item of the PNDS, the percentages of the qualitative data were calculated, for the quantitative data the mean and the maximum and minimum dispersion indices were calculated. Results: 46 patients were included. Concerning the search for associated malformations, 12.1% of patients had a complete assessment before being sent to the university hospital. The biological assessment was complete before the first consultation for 14.6% of patients. Patients had recourse to vaginal reconstruction in 68.3% of cases, of which 39.3% benefited from surgical vaginal reconstruction without information on a prior trial of the Franck technique or not. Fertility preservation had not been offered to any patient before their first visit to the university hospital. One patient had benefited from a genetic consultation and 2 patients from a karyotype during their initial visit outside the university hospital. Conclusion: Given the frequency of this pathology, which is not extremely rare and can impact the quality of life of affected women, it seems important to inform health professionals, particularly gynecologists and general practitioners, about the diagnostic assessment to be carried out in order to optimize patient care. At Rennes University Hospital, an ENT consultation with an audiogram should be offered to patients systematically.
