Nous disposons aujourd’hui de différents outils : l’index de Link, le diagramme de Reiber, et la recherche de bandes oligoclonales d’immunoglobulines G dans le liquide cérébrospinal par isoélectrofocalisation (méthode de référence). Récemment, un nouveau marqueur a été développé et semble présenter un intérêt dans le diagnostic de la SEP : l’index Kappa. L’objectif de ce travail est d’évaluer la pertinence de la prescription des outils actuellement utilisés pour mettre en évidence une SIT d’IgG en regard de la clinique, et d’estimer les forces et les limites de ces outils, afin d’évaluer quelle pourrait être la plus-value de l’utilisation de l’index Kappa dans notre pratique quotidienne. Méthode : Etude rétrospective avec recueil de données et analyse des résultats de 249 patients pour lesquels une prescription de recherche de synthèse intrathécale d’IgG a été faite au laboratoire de Biochimie du CHU de Rennes entre 04/11/2022 et le 27/03/2023. Résultats : D’après nos résultats, il existe une faible corrélation entre les 3 techniques dispo-nibles au laboratoire pour la recherche de synthèse intrathécale d'IgG (isoélectrofocalisation, index de Link, diagramme de Reiber). De plus, la prescription de cet examen ne semble pas être l'outil diagnostique le plus pertinent en dehors d’une suspicion de pathologie neuro-inflammatoire, ce qui représentait plus de 60% des prescriptions. Conclusion : Ces résultats nous permettent de poser un regard critique sur nos pratiques, et de réfléchir à la mise en place d’une revue de prescription et de la réorganisation de la validation biologique de l’isoélectrofocalisation. L’évaluation de l’index kappa nous apparaît pertinente dans notre population de patients au CHU de Rennes. Une prochaine étude aura pour objectif principal d’évaluer l’intérêt de l’index kappa pour la prise en charge diagnostique et pronostique de patients avec pathologie neuro-inflammatoire et/ou suspicion de SEP. Context : The demonstration of intrathecal immunoglobulin G synthesis is part of the diagnosis of inflammatory diseases of the central nervous system, including multiple sclerosis. Today we have different tools : Link index, Reibergram, and oligoclonal band detection by isoelectric focusing (reference method). Recently, a new marker has been developed and seems to be of interest in the diagnosis of multiple sclerosis : the Kappa free light chains index. The aim of this work is to evaluate the relevance of the prescription of tools currently used to demonstrate intrathecal immunoglobulin G synthesis compared to the clinical situation, and to assess advantages and drawbacks of these tools, in order to evaluate what could be the added value of using the Kappa free light chains index in our daily practice. Method : Retrospective study with data collection and analysis of the results of 249 patients who had a prescription of research for intrathecal IgG synthesis, made in the Biochemistry laboratory of Rennes University Hospital between 04/11/2022 and 03/27/ 2023. Results : According to our results, there is a weak correlation between the 3 methods available in the laboratory for the research for intrathecal immunoglobulin G synthesis (isoelectric focusing, Link index, Reibergram). Furthermore, the prescription of this analysis does not seem to be the most relevant diagnostic tool apart from a suspicion of an inflammatory disease of the central nervous system, which represented more than 60% of prescriptions. Conclusion : These results enable us to take a critical look at our practices, and to reflect on the implementation of a prescription review and the reorganisation of the biological validation of isoelectric focusing. The assessment of the Kappa free light chains index seems to be relevant in our population of patients at Rennes University Hospital. The main objective of a future study will be to evaluate the interest of the Kappa free light chains index for the diagnostic and prognostic management of patients with inflammatory disease of the central nervous system and/or suspected multiple sclerosis.
