Cependant, il n’existe pas de recommandations concernant le traitement optimal de ces patients. L’objectif de cette étude rétrospective monocentrique a été d’évaluer le devenir fonctionnel de l’ensemble des occlusions en tandem traitées par thrombectomie mécanique, un devenir fonctionnel favorable étant défini comme un mRS ≤ à 3 mois. Patients et méthodes: Parmi 987 patients avec accident vasculaire cérébral ischémique recent ayant bénéficié d’une thrombectomie mécanique entre le 01/01/15 et le 01/10/19, 37 patients avec lésions en tandem ont été sélectionés : 16 ont bénéficié d’une thrombectomie seule et 21 ont bénéficié d’une thrombectomie associée à un traitement carotidien. Résultats: Les occlusions en tandem ayant bénéficié d’un traitement carotidien apparaissent significativement associées à un mauvais devenir fonctionnel en comparaison à celles ayant bénéficié d’une thrombectomie seule. On ne note pas de sur-risque hémorragique après thrombolyse intra-veineuse. La recanalisation intra-cranienne apparait similaire entre les deux groups (73.3% de mTICI 2b ou 3 pour le groupe avec traitement carotidien vs. 71.4% pour le groupe avec thrombectomie seule). Conclusion: Notre étude n’a pas permis de montrer la supériorité de la thrombectomie associée au traitement carotidien dans le cadre d’une occlusion en tandem. Cependant, nous n’avons pas relevé de complication en lien avec la technique. La nécessité d’une étude clinique interventionnelle est critique dans un future proche. Importance: Tandem occlusions account for approximately 10-15% of acute ischemic strokes caused by large vessel occlusion. However, there is no standard of care regarding the optimal interventional approach for these lesions. Objective: The aim of this study was to analyze the clinical outcomes of all stroke patients with severe tandem lesions treated with mechanical intracranial thrombectomy. Design: We conducted a single-center retrospective study based on all stroke patients with severe tandem lesions treated by mechanical intracranial thrombectomy at our comprehensive stroke center between January 2015 and October 2019. Participants: Out of 987 patients with acute ischemic stroke, 37 patients with severe tandem lesions treated by mechanical intracranial thrombectomy were selected. Sixteen of these patients underwent intracranial thrombectomy alone and the other 21 underwent intracranial thrombectomy with extracranial ICA lesion treatment. Main outcome: The primary outcome was a favorable outcome as defined by an mRS score of ≤ 2 at 3 months’ follow-up. Results: Extracranial ICA lesion treatment with mechanical thrombectomy was associated with poor outcome (3-month mRS >2) compared to mechanical thrombectomy alone. Pretreatment intravenous thrombolysis was not associated with increased risk of hemorrhagic complications. No difference in the intracranial recanalization rate was seen between the two groups (73.3% with an mTICI score of 2b or 3 in the extracranial ICA lesion treatment group vs. 71.4% in the mechanical thrombectomy alone group). Conclusion: Our study failed to demonstrate the superiority of extracranial ICA lesion treatment over mechanical thrombectomy alone. However, the safety of the technique appears acceptable. There is a pressing need for a randomized interventional clinical trial in the near future.
