Insuffisance mitrale Remodelage ventriculaire Médecine--Appareils et matériel Échocardiographie Objectif : Analyser l'impact du Mitraclip (MC) sur la fonction et le remodelage des cavités cardiaques en examinant les changements dans les paramètres de suivi du speckle myocardique, en fonction des caractéristiques pré-interventionnelles des patients. Méthodes : Etude observationnelle rétrospective monocentrique sur des patients ayant bénéficié de la pose de MC entre juillet 2018 et octobre 2020. Nous avons inclus les patients ayant une échocardiographie disponible avant la procédure et à distance, et encore en vie en avril 2021. Les ETT ont été analysées selon une approche standardisée. Nous avons effectué un regroupement hiérarchique en fonction de la variation du volume télédiastolique du ventricule gauche (VTDVG) et du ITG (indice de travail global) avant et après MC. Résultats : 80 patients ont été inclus dans l'analyse (50 présentaient une insuffisance mitrale (IM) primaire, 30 une IM secondaire). 4 clusters ont été obtenus. Le cluster 1 regroupe des patients présentant une diminution significative du VTDVG (-12,1% [-15,8 ; -2,54],(P<0,0001)) et une dégradation significative du strain global longitudinal (SGL) (-9,29% [-23,7 ; -3,40](P=0,0003)). Dans le groupe 2, on observe une réduction non significative du VTDVG mais une amélioration significative du SGL (+24.3 [12.6; 73.8] (P=0,0003)). Le cluster 3 rassemble surtout des patients avec des IM primitives, avec une diminution significative du VTDVG (-52,7 [-67,3 ; -41,6] (P<0,0001), mais aucune amélioration des paramètres de déformation. Le cluster 4 regroupe principalement des patients avec des IM secondaires, ayant une augmentation du LVEDV (+ 23.2 [14.8 ; 33.8] (P<0.0001)), sans amélioration des paramètres de strain. Conclusion : Le remodelage du ventricule gauche après l’implantation de MC n'est pas uniforme. En regardant les paramètres de déformation et les volumes, nous proposons 4 groupes ayant des caractéristiques et une réponse différente au MC. Introduction: Mitral valve regurgitation (MR) is still a matter of understanding for best select candidates for treatment. Nowadays, Transcatheter edge-to-edge repair (TEER) is part of the routine. Consequences of TEER on heart remodeling is difficult to anticipate. Markers of anatomical and functional remodeling can be assessed by echocardiography. Therefore, we sought to analyze the impact of TEER on cardiac chambers’ function and remodeling looking at volumes and myocardial work indices. Methods: This single-center retrospective observational study selected and analyzed patients who underwent TEER between July 2018 to October 2020, at Rennes University Hospital. Patients with a pre-TEER and follow-up exams available, still alive in April 2021, were included. Echocardiographies were analyzed according to a standardized approach. Pre-intervention and follow-up clinical and echocardiographic data were used for displaying the impact of TEER on cardiac chambers changes after correction of MR. We performed a hierarchical clustering according to annualized changes in LDVEDV and GWI (global work index) from pre to post-TEER. Results: 80 patients were enrolled (50 presented primary MR, 30 secondary MR). 4 clusters were suggested. Cluster 1 gathers patients with a significant reduction in LVEDV (-12.1% [-15.8; -2.54] (P<0.0001)) and a significant degradation in global longitudinal strain (GLS) ( -9.29% [-23.7; -3.40] (P=0.0003)). In Cluster 2, a nonsignificant reduction of LVEDV but a significant improvement in GLS (+24.3 [12.6; 73.8] (P=0,0003)). Cluster 3 gathers mostly patients with primitive MR, with a significant decrease in LVDEV (-52.7 [-67.3; -41.6] (P<0.0001), but no improvement in deformation parameters. Cluster 4 gathers mostly patients with secondary MR, having an increase in LVEDV (+ 23.2 [14.8; 33.8] (P<0.0001)), without improvement in deformation parameters. Conclusion: LV remodeling post-TEER is non uniform. Looking at deformation parameters and volumes we propose 4-groups having different characteristics and response to TEER.
