Le but de cette étude est de savoir s’il serait un indicateur utile durant la circulation extra-corporelle (CEC) afin d’adapter le débit au besoins métaboliques du patient. Méthode : Il s’agit d’une étude rétrospective monocentrique ayant inclue 178 patients qui ont bénéficiés d’une CEC de plus de 90min entre Mars et Octobre 2019 au CHU de Rennes. Le PCO2 gap était mesuré au début de la CEC. La limite de 6mmHg a été utilisée pour définir les 2 groupes PCO2 gap élevé et normal. Le critère principal était un critère composite incluant hyperlactatémie, VIS20, élévation de la créatinine et mortalité hospitalière. Résultats : la mauvaise évolution était retrouvé dans 45,3% et 42% des cas dans les groupes PCO2 gap normal et élevé. Conclusion : Le PCO2 gap mesuré durant la CEC ne semble pas corréler à la mauvaise évolution ultérieure. Introduction : PCO2 gap is a marker of cardiac output, used in high risk surgical patient, septic shock as well as in cardiac surgery patients. The goal of the study is to know whether it can be useful to adjust the flow rate during cardiopulmonary bypass (CPB). Methods : We conducted a retrospective study. 178 patients who underwent a CPB90min were included between March and October 2019. The PCO2 gap was measured in the begining of CPB and two groups were compared : High PCO2 gap  6mmHg and normal < 6mmHg. The primary endpoint was a composite of lactatemia, VIS20, elevated creatinine and in-hospital mortality. Results : Poor outcome occurred in 45.3% of the normal PCO2 gap and 42% in the High PCO2 gap group. Conclusion : PCO2 gap measured during CPB doesn’t seem to be correlated to pour outcome in the ICU.
