Voies biliaires Cancer Chirurgie Radiothérapie interne sélective Traitement néoadjuvant Études comparatives Introduction : Le cholangiocarcinome intra-hépatique (CIH) est un cancer découvert souvent à un stade tardif, dont le pronostic est péjoratif en l’absence de chirurgie, seul traitement reconnu à visée curatrice. La place des traitements locorégionaux est mal définie à ce jour. Le but de cet étude était de comparer le devenir des patients atteints de CIH opérés d’emblée ou après traitement de downstaging. Méthodes : Nous avons mené une étude rétrospective unicentrique incluant les patients opérés d’un CIH entre janvier 1997 et novembre 2017 à Rennes. Deux groupes ont été comparés, patients opérés d’emblée versus patients opérés après traitement de downstaging par chimiothérapie ou radioembolisation. Nous avons analysé la survie globale et la survie sans progression entre ces groupes. Résultats : 169 patients ont été inclus, 137 dans le bras chirurgie d’emblée et 32 dans le groupe downstaging, considérés non résécables. Parmi ces 32 patients, 13 ont reçu de la chimiothérapie seule et 19 de la radioembolisation associée le plus souvent à de la chimiothérapie. La médiane de survie globale n’était pas significativement différente entre les deux groupes : 32,3 mois [IC95% : 23,9-40,7] pour le groupe chirurgie d’emblée versus 45,9 mois [IC95% : 32,3-59,4] pour le groupe downstaging (p=0,537). En analyse multivariée, le groupe downstaging n’était pas associé à un meilleur ou moins bon pronostic. Cependant, en analyse multivariée la REL était associée à un bénéfice significatif HR = 0,34 [IC95% : 0,14-0,84] (p=0,019). Conclusions : Cette étude montre que la survie globale des patients traités par chirurgie après traitement de downstaging est comparable à celle des patients opérés d’emblée et incite à faire reconsidérer l’opérabilité des patients jugés initialement non résécables après traitement médical premier. La REL pourrait être un traitement néoadjuvant intéressant chez les patients porteurs de CIH difficilement résécables en améliorant les résultats chirurgicaux. Background: Intrahepatic cholangiocarcinoma (ICC) are tumors with poor prognosis because of delayed diagnosis. Complete surgical resection remains the only potential curative treatment. Locoregional treatments are not well defined in those situations. The aim of this study was to compare the outcomes of patients resected for ICC with upfront surgery or after downstaging treatment. Methods: All consecutive patients with ICC between January 1997 and November 2017 were included in a single-center database and retrospectively reviewed. Patients were divided in two groups: upfront resection or resection after downstaging using either chemotherapy alone or selective internal radiation therapy (SIRT), combined with chemotherapy. Overall survival (OS) and recurrence free survival (RFS) were compared between those groups. Results: 169 patients resected for ICC were included. 137 underwent upfront surgery and 32 first received downstaging treatment because their tumor was initially unresectable. Median OS was not different between the 2 groups: 32·3 months [95%CI : 23·9-40·7] with primary surgery versus 45·9 months [95%CI : 32·3-59·4] with downstaging treatment (p=0·537). In multivariable analysis, when downstaging treatment was entered in the model along with variables associated with OS in univariable analysis, downstaging treatment was not associated with a better or worse prognosis. However, delivery of SIRT was associated with a significant benefit in multivariable analysis, HR=0·34 [95%CI : 0·14-0·84] (p=0·019). Conclusions: Overall survival of patients resected after downstaging treatment was not different compared with the OS of patients resected upfront. SIRT may be an efficient neoadjuvant therapy in patients with unresectable ICC, in order to improve surgical results.
