Le pronostic des patients en traitement de suppléance de l’IRCT secondaire à une sclérodermie est mal connu. En France, 120 patients atteints de sclérodermie ayant débuté un traitement de suppléance de l’IRCT entre 2002 et 2014 ont été inclus dans le registre REIN (Réseau Epidémiologie et Information en Néphrologie). En analyse multivariée, la sclérodermie est associée de façon significative avec une sur-mortalité (HR 1.76; 95% CI 1.27-2.45; p= 0.001). Les patients atteints de sclérodermie en IRCT ont été comparés à 240 témoins appariés : en analyse multivariée, la sclérodermie est associée de façon significative avec une sur-mortalité (HR 1.76; 95% CI 1.27-2.45; p= 0.001). Les patients atteints de sclérodermie avaient moins accès à la liste d’attente de transplantation rénale (HR 0.36; IC 95% 0.23-0.56, p<0.001) et à la greffe (HR 0.28; 95% CI 0.15-0.52; p< 0.001) que les témoins appariés. La sclérodermie est un facteur indépendant de mortalité et d’un moindre accès à la transplantation rénale chez les patients en IRCT. Scleroderma renal crisis (SRC), the most frequent cause of renal complication of systemic sclerosis (SSc) can lead to end stage renal disease (ESRD). The main objectives of our study were to describe, from data extracted from the French Renal Epidemiology and Information Network (REIN) registry, characteristics, and outcomes in incident French cohort of patients on dialysis due to SSc compared with matched patients. Between 2002 and 2014, 120 incident SSc patients started RRT in France. SSc was significantly associated with higher mortality (HR 1.76; 95% CI 1.27-2.45; p= 0.001) in comparison with matched controls. Dialysis withdrawal was reported in 22 (18.3%) of SSc patients compared with 15 (6.3%) of the controls. Patients with systemic sclerosis have less access to waiting list (HR 0.36; IC 95% 0.23-0.56, p<0.001) and to kidney transplantation (HR 0.28; 95% CI 0.15-0.52; p < 0.001). Prognosis of patients with ESRD from SSc is still poor but registration on transplant waiting list should be more often considered for the youngest scleroderma patients with less comorbidities.
