To a remarkable degree, the history of cancer services shows the accumulation of different modalities, especially surgery, radiotherapy and chemotherapy. This review asks about the structural features of these different modalities and the ways in which new ones were added -sometimes in contestation with the old. It explores how conditions in different nations favoured different modalities, and the subsequent effects on patterns of services.
