Cependant, depuis 2009, une quinzaine de cas ont rapporté l’émergence in vivo d’une résistance de haut niveau à la daptomycine chez Corynebacterium spp. Ce phénomène, rare mais probablement sous-estimé, survient en moyenne après 20 jours de traitement et semble favorisé par certains facteurs comme le recours à la daptomycine en monothérapie, à une posologie insuffisante et de façon prolongée. Concernant le mécanisme sous-jacent, des altérations du gène pgsA2, impliqué dans la synthèse du phosphatidylglycérol, un composant essentiel de la membrane bactérienne et cible de la daptomycine, ont été identifiées chez plusieurs souches résistantes. Les trois cas supplémentaires étudiés confirment et enrichissent les données existantes et rappellent ainsi la remarquable capacité d’adaptation de C. striatum renforçant son caractère de pathogène opportuniste préoccupant, notamment dans le contexte hospitalier marqué par la fréquence croissante des infections associées aux soins. Bien que la daptomycine conserve une place essentielle dans l’arsenal thérapeutique, son utilisation requiert une vigilance particulière et requiert des recommandations précises concernant les posologies, les durées de traitement et l’intérêt des associations. Résumé (anglais) : Daptomycin is a naturally occurring cyclic lipopeptide whose unique bactericidal activity and favorable tolerance profile have gradually established it as a valuable therapeutic alternative in the treatment of infections caused by Corynebacterium spp., although this indication is not officially recognized. However, since 2009, around 15 cases have reported the emergence of high-level resistance to daptomycin in Corynebacterium spp. in vivo. This phenomenon, which is rare but probably underestimated, occurs on average after 20 days of treatment and appears to be favored by certain factors such as the use of daptomycin as monotherapy, at an insufficient dosage and for a prolonged period. Regarding the underlying mechanism, alterations in the pgsA2 gene, involved in the synthesis of phosphatidylglycerol, an essential component of the bacterial membrane and target of daptomycin, have been identified in several resistant strains. The three additional cases studied confirm and enrich the existing data and thus highlight the remarkable adaptability of C. striatum, reinforcing its status as a worrying opportunistic pathogen, particularly in the hospital setting, where healthcare-associated infections are becoming increasingly common. Although daptomycin remains an essential part of the therapeutic arsenal, its use requires particular vigilance and specific recommendations regarding dosages, treatment durations, and the benefits of combinations.
